CHAMPS Instructions & Information

New individual caregivers must have their enrollment approved through the online MDHHS CHAMPS Provider Enrollment (PE) subsystem to be reimbursed for covered services rendered to eligible Medicaid beneficiaries. A provider's participation in Medicaid will be effective on the date the provider's online application is submitted, or a provider may request that enrollment be retroactive to a specific date when completing the online application.

New individual caregivers can indicate in their enrollment if they would like to be added to a provider registry list which is shared by MDHHS local county office with new beneficiaries needing home help services.

Changing or Updating CHAMPS Information

Providers must notify MDHHS within 35 days of any change to their enrollment information. Below are instructions on how to update some of the more frequently updated enrollment items. Note updating or changing CHAMPS information is also referred to as a CHAMPS modification.

Revalidation

All providers are required to revalidate their Medicaid enrollment information a minimum of once every five years, or more often if requested by MDHHS. MDHHS will notify providers when revalidation is required.
